1. How to become an IVF Specialist in India?

   Reproductive medicine and surgery are a subspecialty branch in obstetrics and gynaecology which deals with management of infertile couples, reproductive health problems, fertility preservation etc. It includes the practice of ART or assisted reproductive techniques which include IUI, IVF etc. It also involves surgeries for reproductive health, laparoscopic and hysteroscopic surgeries. You also have to be an endoscopy expert as there will be a lot of ultrasonography’s that you will be doing during your IVF practice.
   Qualifications required for an infertility specialist:
   After your MBBS you have to do post-graduation in OBG like MD, MS or DNB, MD and then you have to sit for exams like NEATSS, INISS and FET. With these exams you get admissions in courses like DM, MCH, FNB and FRM. These are the certified courses available in our country and according to the ART Act you have to work under someone in an IVF hospital for three years or complete 50 OPU’s before you start your own practice.

   If you are a busy practitioner and have hardly any time for continued medical education, you still have a solution to complete a Fellowship in Reproductive Medicine. If you don't want to take the exams, the other way to do IVF fellowships is to find the nearest busiest IVF centre in a major city near you, go to the hospital and ask about their fellowship programs or search the website and make an online inquiry.

You will be given a chance to observe the rarest of rare cases and if you are lucky enough, the experts may even give you a hands-on. But this depends directly on the merit of the student. Further, during the practical training session in the clinic, you will also be given cases for OPU’s and as per the ART guidelines, you will also be given the 50 OPU certification which will be serving the requirement of opening ART Level 1 clinic in India.
   To be eligible for individual practice or opening up an IVF clinic in Bangalore, it is mandatory that you work under someone. Medline Academics and Dr. Kamini Rao Hospitals, opens this opportunity for students a full-time Fellowship Program as an addition to the existing hybrid program.
   A lot of obstetricians choose IVF with the hope that they will be able to discontinue their obstetric practice which becomes tiresome for some people because there are so many emergencies. In fact, entire obstetrics is just emergencies only. So, a lot of IVF practitioners have stopped their routine practice. But you all must have heard that this field of reproductive medicine is currently super saturated and the new practitioners who are joining this field are not only doing IVF but also doing at least obstetric management of their own IVF patients to increase the amount of practice that they are doing. So, it is your choice whether you want to become a good established IVF practitioner, you may as well leave obstetric practice but a lot of people continue to do both OBG and IVF.
   So don't do IVF with the hope that you will be able to completely do away with obstetric practice. It also depends on your interest whether you are interested in the medicine part also there will be a lot of surgeries to be done. You will also have to learn laparoscopy.
   Let’s Understand IVF
   IVF is in vitro fertilization wherein we stimulate the ovaries by giving hormonal injections to produce multiple follicles and from these follicles we extract the fluid and identify the eggs which are present in the follicles under high resolution microscope. These eggs are then fertilized with the partner ***** either with conventional IVF or with something called as ICSI or ICSI in which each egg is injected with the **. Now, this ensures fertilization.
   After fertilization, these embryos are then kept inside incubators in the lab and grown in the lab for 3 to 5 days so that the development takes place further. And after the embryos are properly developed, they are again transferred back into the woman's *** or womb. And after transfer, we give supportive treatment for 14 to 16 days so that the pregnancy, if it happens, then it is supported by these hormones and other injections.
   Now, this is something which we call as self-IVF. There are conditions in which either the woman cannot produce the eggs which are there so you take help of a donor. Now, this could be in slightly older group of women or even in younger women sometimes when your ovaries have stopped producing eggs or the number of follicles or eggs which you have are very poor.
   In those cases, we take help of eggs from younger women and fertilize those eggs with the partner ***** and transfer the embryo back into the woman's ******. And this is something program called as donor egg IVF program.
   Current Scenario

   In the olden days, there were stigmas attached to the fact that a woman does not bear a child. They were neither allowed for rituals, nor for any functions. But today there’s a lot of improvement.    There was a WHO statement which came out in the late 20s that the infertility incidence is 17.5% in LMICs. It's so high.
   Causes?
   • The awareness has increased
   • Girls are getting educated, they're delaying childbearing, they're getting married late.
   • Sexually transmitted diseases are on the rise
   • Because of the pollution, the contamination, we are seeing a lot of young girls with accelerated ovarian aging. The girl may be 28 years old, but her ovary is 40 years old.
   • Lifestyle factors like smoking, drinking etc.
   Surrogacy – What you should be knowing
   Surrogacy actually is meant when the girl cannot conceive, but not a lifestyle option. Pregnancy comes with its own complications, with its own problems. And just because you don't want to get pregnant, you're putting somebody else at risk. The ART law has finally streamline this process and nobody can play with it now.
   There are three tiers of IVF centres:
   • One is the ones with the army,
   • The ones with the medical colleges,
   • The ones in municipal and government hospitals like AIMS, D.Y. Patil, etc.
   Those are the ones which have low budget IVF. And they do help patients, they get, you know, medicines at a lower cheaper cost. So, it is accessible.
   What is the emotional trauma about infertility that no one talks about?
   It is traumatic. Anxiety is maximum. Being an IVF fertility doctor and a gynaecologist is not only about doing the procedure, but also somewhere being a psychologist because you're helping them.
