FYI.. your auto form wouldn't accept my NAME as it appears on the card...
When I put my name as it appears on my card in the line, it would NOT accept it as it is printed on the card. I had to alter it and it does NOT represent how the name is on the card but it's the only way your system would accept the payment...

remove the 5 digit criteria for postcode. Not all postcodes have 5 digits. I had to stick a 0 in front of mine for it to be accepted

Don't erase entered data if the user switches from Debit to Credit Card
The Debit tab is the default. If the user enters their card information and then realizes later that it is actually a Credit Card that will be used, the fields are cleared if the user then clicks on the Credit Card tab. Don't clear out already entered information if the user clicks on another tab, so that the user doesn't have to re-enter it again.
