  1. Your form should make it much more clear that the user is choosing between "debit card" and "credit card." It marked "debit," not "credit."

    Agree. Dont show payment fields until user explicitly selects between debit, credit, and bank

  3. Fix your software so that the downloaded receipt prints the correct invoice number
